How can I use checkboxes in a HTML form for selecting a download?

Hi there, I'm trying to create a HTML form which will send me an email containing the details the user inputs (name and email address) but also have 5 checkboxes in the form which they can tick to download the relevant files. I would also like to be sent in the email which files they have downloaded. How am I able to do both functions? I have got the email part working, I just don't know how to initiate the downloading function... Any help would be greatly appreciated.
